Location: AnMed Health North Campus, 2000 E. Greenville St., Ste. 2400, Anderson, SC Details: Series of 3 diabetes self-management classes. Coveres blood glusose monitoring, carb counting, exercise, medication and pyschosocial issues. Each class offered daytime only, twice per week. Individual Sessions also available. Location: Pee Dee Diabetic Consultants In Piggly Wiggly Pharmacy Details: 10 sessions offered- with 1 one on one session. Offered on Tuesday afternoon, Wednesday morning of afternoon and Thursday morning or afternoon. Covers carbohydrate counting, testing,medications,exercise,sick day rules,lab results,weight control,complications. Sessions available by doctor referral. Location: Palmetto Health Baptist, Columbia, SC Details: Nationally recognized by the American Diabetes Association, the Outpatient Diabetes Education Program of Palmetto Health Baptist provides quality, comprehensive Diabetes Education. Instructors include Certified Diabetes Nurse Educators and Registered Dietitians who are also Certified Diabetes Educators. Medical Social Workers are available to address financial concerns and social support. For patients with Type 2 diabetes, the program includes an initial assessment and three diabetes group classes (3 hours each)and a follow up appointment. Individual diabetes education classes are available for patients with Type 1 diabetes and some patients with Type 2 diabetes. All instruction is based on the Clinical Practice Recommendations of the American Diabetes Association. MD referral is required for participation. Most insurance plans cover a percentage of Diabetes Education. Classes include carbohydrate counting, medications, physical activity, monitoring, general diabetes overview, medical nutrition therapy, stress, coping, and community resources.
